Treatment-Resistant Schizophrenia: New Guidelines on Diagnosis and Terminology

February 21, Schizophrenia Research Forum held a discussion about new guidelines on treatment-resistant schizophrenia. Oliver Howes of the Institute of Psychiatry in London, and Christoph Correll and John Kane of Zucker Hillside Hospital, Northwell Health, Glen Oaks, New York, were the prime movers behind the Treatment Response and Resistance in Psychosis (TRRIP) Working Group, which published its consensus statement recently in the American Journal of Psychiatry (Howes et al., 2017).

In a series of meetings at schizophrenia conferences over the past few years, the TRRIP group has grappled with the lack of standard criteria for determining that a person is resistant to treatment with antipsychotic medications. This problem affects 1) research, where inconsistent criteria make meta-analyses difficult and produce conflicting results for medication efficacy, and 2) clinical care, where patients may not receive optimal care. The panelists will present their methodology for developing consensus guidelines and general principles of defining treatment-resistant schizophrenia, as well as their recommendations for determining and reporting treatment resistance or response.
